In Michaelis–Menten terms, what happens to the apparent Km and Vmax of hepatic HMG-CoA reductase once a statin-type competitive inhibitor occupies the active site, and why does this set a ceiling on dose escalation?

A competitive inhibitor vies for the same active site, so a higher substrate concentration is needed to reach half-maximal velocity—the apparent Km rises while Vmax stays unchanged, since saturating substrate can still occupy every enzyme molecule. On a Lineweaver–Burk plot the lines pivot through a shared y-intercept. Because Vmax is capped, escalating statin doses approach but never exceed maximal suppression of the enzyme, so returns diminish with each increment.

Why does a competitive inhibitor raise the apparent Km of an enzyme without changing Vmax, and how is this pattern recognized on a Lineweaver-Burk plot?

A competitive inhibitor resembles the substrate and binds reversibly in the active site, sequestering part of the enzyme as an EI complex. Raising substrate concentration outcompetes the inhibitor, so at saturating S the full catalytic rate is restored and Vmax is unchanged; more S is simply needed to reach half-maximal velocity, giving Km(app) = Km(1+[I]/Ki). On the Lineweaver-Burk plot all lines intersect at the same y-intercept (1/Vmax), while slopes increase by the factor (1+[I]/Ki).

What does the Michaelis constant Km physically represent, and can its value be used directly to compare substrate affinity across different enzymes?

Numerically, Km equals the substrate concentration at which velocity reaches half of Vmax; rigorously it is defined as (k-1+kcat)/k1, the sum of ES breakdown rates divided by its formation rate. Only when k-1 greatly exceeds kcat—that is, the complex is more likely to dissociate before catalysis—does Km approximate the dissociation constant Kd, and only then does a lower Km indicate tighter binding. When kcat contributes substantially, Km deviates from Kd, so comparing Km values across different enzymes can mislead; the safe reading applies within one enzyme-substrate system or when the rapid-equilibrium assumption holds.
