How does affinity maturation in the germinal centre proceed through repeated cycles of mutation and selection?

Germinal centres are split into a dark zone where B cells divide rapidly while the cytidine deaminase AID introduces somatic hypermutation into immunoglobulin variable regions, and a light zone where the cells compete for antigen displayed on follicular dendritic cells and for help from follicular helper T cells. Clones with higher affinity capture more antigen and receive stronger help signals such as CD40L and IL-21, so they are selectively expanded and cycle back to the dark zone for further mutation. This iterative loop is Darwinian selection inside the body, yielding high-affinity antibodies and memory cells.

How do MHC class I and class II presentation pathways differ, and why is cross-presentation crucial for antiviral and antitumour immunity?

MHC class I molecules mainly load peptides from proteasome-degraded cytosolic proteins, delivered into the endoplasmic reticulum by TAP and shown to CD8-positive cytotoxic T cells, whereas class II molecules load fragments of exogenous proteins in endolysosomal compartments for CD4-positive helper T cells. Cross-presentation is the ability of dendritic cells to route internalized exogenous antigen into the class I pathway, so cytotoxic responses can be primed even when the presenting cell is not itself infected or transformed. This route underlies CD8 priming by most viral vaccines and by therapeutic cancer vaccines.

Why does replacing uridine with modified nucleosides such as pseudouridine both lower inflammation and raise protein output from mRNA?

Unmodified single-stranded RNA is read as a danger signal by innate sensors including TLR7, TLR8, RIG-I and PKR, triggering interferon responses while PKR phosphorylates eIF2-alpha and shuts down translation initiation. Substituting uridine with pseudouridine or N1-methylpseudouridine sharply reduces sensor recognition and inflammatory cytokine release. Because translation is no longer blocked and transcript stability improves, protein yield per molecule can rise by roughly an order of magnitude, which was decisive for making mRNA vaccines practical.

Why does measles vaccination provide protection for decades after only one or two doses, while influenza vaccines need their formula updated every year?

Measles virus keeps a highly conserved antigen profile, and the live attenuated vaccine replicates briefly in the body, sustaining germinal-center reactions that generate high-affinity memory B cells and long-lived plasma cells able to persist in bone marrow for decades. Influenza virus instead alters its hemagglutinin and neuraminidase epitopes rapidly through antigenic drift, so established immune memory struggles to recognize new strains. The World Health Organization therefore redefines vaccine composition annually based on predictions drawn from global surveillance data.

Which cell types carry the immune memory that a vaccine leaves behind?

Three populations dominate: long-lived plasma cells in the bone marrow that keep secreting antibody as a first barrier, memory B cells that reactivate and can undergo further affinity maturation on re-exposure, and memory T cells in central, effector and tissue-resident forms that re-expand or clear infection locally. A waning antibody titre therefore does not equal lost protection, because memory cells can rebuild a high-titre response within days of re-infection.

Why do mRNA vaccines need to be packaged in lipid nanoparticles to work effectively?

Naked mRNA is large and negatively charged, so it is rapidly degraded by extracellular ribonucleases and cannot cross the similarly charged plasma membrane. Lipid nanoparticles built from ionizable lipids, helper phospholipids, cholesterol and PEG-lipids shield the mRNA, enter cells by endocytosis, and release the cargo into the cytosol when the endosome acidifies. The lipid carrier also provides intrinsic adjuvant activity that strengthens innate immune activation.

How is the herd immunity threshold estimated, and why does it vary so much between diseases?

In a simple homogeneous-mixing model the threshold is approximately one minus the reciprocal of the basic reproduction number, so the more transmissible the pathogen the larger the immune fraction required. Measles, with a basic reproduction number often quoted at 12 to 18, needs over 90 percent immunity, while less transmissible infections such as seasonal influenza need much less. Real thresholds also depend on uneven vaccine coverage, contact structure, waning protection and pathogen evolution, so the formula is only a first approximation.

What is the key difference between live-attenuated vaccines and mRNA vaccines in how they establish immune memory?

Live-attenuated vaccines replicate in a limited fashion, keeping antigen expressed in lymphoid tissue for weeks, so B cells along with CD4 and CD8 T cells are all broadly engaged and the resulting memory resembles natural infection. mRNA vaccines do not replicate; host cells translate the antigen only transiently, so memory rests mainly on dendritic-cell cross-presentation and germinal-center reactions, and boosters are usually needed to expand the memory pool. The former gives broader immunity but requires caution in immunocompromised recipients, while the latter is simple in composition and quick to redesign.

How do innate and adaptive immunity differ in the way they recognize pathogens and in their response speed?

Innate immunity relies on pattern-recognition receptors such as Toll-like receptors that detect conserved molecular patterns shared by microbes; it acts within minutes to hours but leaves no memory. Adaptive immunity uses somatically rearranged B-cell and T-cell receptors to recognize specific epitopes, requiring days for clonal expansion but generating durable memory. The two arms are coupled: antigen presentation and costimulatory signals from innate cells set the magnitude and flavour of the adaptive response.

By what mechanisms do vaccine adjuvants strengthen the immune response?

Adjuvants act in two broad ways: they prolong antigen retention at the injection site and promote uptake and transport of antigen by dendritic cells to lymph nodes, and they directly engage innate pathways to supply cytokine and costimulatory context. Aluminium salts mainly boost antibody responses and are linked to inflammasome activation and local damage signals, whereas AS01-type adjuvants combining the TLR4 agonist monophosphoryl lipid A with a saponin markedly enhance T-cell immunity, as used in the recombinant zoster vaccine. Adjuvant choice therefore shapes the quality of immunity, not merely its magnitude.
