How do resistance genes spread rapidly between bacteria — even across species — and what does this mechanism demand of how we use antibiotics?

Resistance genes often ride on mobile genetic elements such as plasmids. By conjugation a bacterium hands a plasmid directly to a neighboring strain; it can also pick up foreign genes through transformation (free DNA uptake) or transduction (by phages), acquiring multidrug resistance in a single transfer. Antibiotic use itself is the selection pressure — susceptible bacteria are cleared, resistant ones expand, and shared human and animal microbiomes carry them across hosts. New drugs alone cannot outrun resistance: antimicrobial stewardship that cuts unnecessary exposure is equally essential.

When a urine culture reports ESBL-producing Escherichia coli, why do clinicians often abandon cephalosporins and switch to a carbapenem instead?

ESBLs are a family of beta-lactamase enzymes produced by gram-negative bacilli that hydrolyze penicillins and nearly all cephalosporins, so infections can fail clinically despite in-vitro susceptibility. The encoding genes usually ride on plasmids and spread horizontally between strains, making ESBL-producing E. coli a major reservoir of resistance in hospitals and the community. For moderate-to-severe infections such as urosepsis caused by these organisms, guidelines favor carbapenems, whose structure resists enzymatic breakdown.

Why does antibiotic misuse accelerate bacterial resistance, even when the person taking them has no resistant infection?

When antibiotics wipe out susceptible bacteria, any cells carrying resistance genes lose their competitors and multiply into dominant flora. Resistance genes also jump between strains on plasmids through horizontal transfer, reaching other commensals and pathogens. Under-dosing, stopping courses early or taking antibiotics casually prolongs this selection pressure, enriching resistant bacteria in people and the environment. Using antibiotics only as prescribed, at full dose and duration, is essential to slow resistance.
