What molecular mechanisms cause bacterial resistance?

Bacteria resist via degrading enzymes, altered drug targets, efflux pumps, and resistance plasmids, and these genes can spread horizontally between species.

What are last-resort antibiotics and why are they protected?

Last-resort drugs like carbapenems treat multi-drug-resistant severe infections; with few alternatives they are tightly restricted to preserve them against total resistance.

In pharmacokinetic/pharmacodynamic terms, why must beta-lactams be dosed in a time-dependent fashion while aminoglycosides allow once-daily high dosing?

The key is which parameter predicts killing best and how strong the post-antibiotic effect is. Beta-lactams show almost no PAE, so efficacy tracks the fraction of the dosing interval with free drug above the MIC (fT>MIC, usually 40% or more), favoring prolonged infusions or shorter intervals. Aminoglycosides are concentration-dependent: killing accelerates as Cmax/MIC rises and a sustained PAE lets one large daily dose hold peaks high while low troughs limit nephrotoxicity and ototoxicity - the basis of stewardship dosing strategies.

Through the lens of cytokine regulation, why does procalcitonin rise steeply in bacterial infections yet usually stay low during viral ones?

Because the cytokine environments the two pathogen classes create switch the same gene in opposite directions. Bacterial endotoxin together with interleukin-1 beta, TNF-alpha and IL-6 drives extra-thyroid tissues such as lung, gut and fat to express CALC-1 and release abundant procalcitonin, whereas the interferon-dominated antiviral response - chiefly IFN-gamma - directly blocks that transcription, keeping levels flat. With a half-life near twenty-four hours and a swift fall once infection is controlled, procalcitonin dynamics let clinicians judge infection type and guide cessation; randomized trials show this safely shortens antibiotic courses.

Why does farm antibiotic use affect human health?

Farm use for growth promotes resistant bacteria in the environment and can pass resistance genes via food chains, so many countries limit non-therapeutic use.

Why must you finish the full antibiotic course?
