Why do clinical AI systems need explainability and prospective validation?

High accuracy on retrospective data does not prove clinical value, because image quality, how clinicians interact with the tool and whether they follow its prompts all change results in a real workflow; prospective studies and ideally randomised trials are needed to measure effects on diagnostic accuracy, turnaround time and patient outcomes. Explainability methods such as saliency maps and feature attribution help clinicians see whether the model relied on implausible cues and create a traceable chain of accountability when it errs. Regulators also require change management for algorithm updates and ongoing performance monitoring after deployment.

Why does a medical AI model often perform much worse at a different hospital?

Models can latch onto shortcut features unrelated to disease, such as scanner model, patient positioning, post-processing style or even markers burned into the image, which happened to correlate with outcomes at the training site and stop working where the data distribution differs. Differences in case mix, prevalence and labelling standards also change positive predictive value, so a tool can become far less useful even when sensitivity and specificity look unchanged. Multi-centre external validation, monitoring of data drift and continuous post-deployment performance tracking are therefore essential.

Which problems suit telemedicine, and when must you go to hospital in person?

Telemedicine works well for chronic disease follow-up and medication adjustment, explaining test results, largely history- or image-based assessment such as dermatology and mental health, and specialist consultation for remote areas, cutting travel and waiting costs. In-person care is required when physical examination, imaging or laboratory tests, emergency treatment are needed or the patient is unstable, for example chest pain, breathlessness, altered consciousness, seizures, major trauma bleeding or high fever with lethargy. It is generally advised to use remote consultation as a complement for triage, follow-up and health management rather than a substitute for all visits.

Why has diabetic retinopathy become one of the most successful uses of AI screening?

The task is well defined and the images are highly standardised: a fundus photograph is graded to decide whether referral is needed, while the number of people with diabetes needing regular screening far exceeds available ophthalmologists. Deep learning algorithms have reached sensitivity and specificity comparable to specialists on large datasets and have entered primary care screening pathways in some countries. Real-world use still depends on image quality control, validation across different populations, and reliable referral and treatment after a positive screen.

How do digital therapeutics differ from ordinary health apps?

Digital therapeutics use the software itself as the intervention, delivering treatment or disease management for a specific condition, and they require clinical trial evidence and regulatory clearance as a medical device or therapeutic product; examples include cognitive behavioural programmes for insomnia, addiction interventions and chronic disease management systems. Ordinary health apps mostly log data, send reminders and present information without making therapeutic claims, and face far lighter regulation. When choosing, it is worth checking the approved indication, the strength of evidence, and whether a prescription or clinical follow-up is required.

How is a medical imaging AI model actually trained?

Researchers first assemble large sets of annotated images, with labels from radiologist reading, pathology results or clinical follow-up outcomes, then train models such as convolutional neural networks to map image features to labels, tuning on a separate validation set and reporting performance on a held-out test set. Typical metrics are sensitivity, specificity and area under the ROC curve, often benchmarked against clinicians. How representative the data are and how accurate the labels are set the ceiling on performance, which is the central bottleneck in medical imaging AI.

Why might large-scale smartwatch screening for atrial fibrillation not improve health outcomes?

Large prospective studies show that irregular-pulse notifications from wearables do identify some previously undiagnosed atrial fibrillation, but only a minority of alerts are confirmed on subsequent ECG patch monitoring, leaving many false positives with attendant testing burden and anxiety. More importantly, screen-detected episodes are often short and low-burden, and randomised trials have not established that anticoagulating them reduces stroke. It is generally advised to treat a device alert as a reason for medical assessment, letting a clinician decide next steps using ECG findings and stroke risk scores.

How can telestroke networks let patients at small hospitals without neurologists still receive timely thrombolysis?

Real-time remote consultation brings stroke expertise from a hub hospital to the bedside at spoke clinics. After the local team uploads a head CT, hub physicians assess the patient over video and review shared images to exclude hemorrhage, weigh indications against contraindications, and decide within minutes whether intravenous thrombolysis is justified; the spoke team then treats immediately. Such hub-and-spoke networks close the specialist gap, avoid transfer delays, and put many more patients on tissue plasminogen activator inside the 4.5-hour window with clearly better outcomes.
