Why can a patient taking an SGLT2 inhibitor such as empagliflozin develop ketoacidosis while glucose is normal or only mildly elevated?

These drugs clear glucose through the urine to lower blood sugar, while reducing insulin demand, raising glucagon and causing osmotic diuretic-driven volume depletion; together these effects accelerate lipolysis and hepatic ketogenesis. With precipitants such as poor intake, surgery or infection, frank ketoacidosis can emerge despite normal glucose: euglycemic DKA. Because the unremarkable glucose delays recognition, stop the drug whenever suspected and treat with intravenous dextrose plus insulin even when glucose is not elevated.

Why is gout linked to uric acid?

Uric acid is the end product of purine metabolism, and when blood levels stay above its solubility limit, monosodium urate crystals precipitate in joints and surrounding tissue. Immune cells sense these crystals and activate the NLRP3 inflammasome, releasing large amounts of interleukin-1beta and producing intense redness, swelling, heat and pain, classically at the base of the big toe. Acute attacks are treated with anti-inflammatory drugs, while long-term care means urate-lowering therapy to a target level plus limiting alcohol (especially beer), organ meats and sugary drinks.

How does the thyroid regulate the body's metabolic rate?

The thyroid secretes T3 and T4 under hypothalamic-pituitary-thyroid control; these hormones enter cells and raise basal metabolism, heat production, and heart rate. Over-secretion causes hyperthyroid palpitations and weight loss, under-secretion causes hypothyroid sluggishness.

How was insulin discovered, and why does it save diabetics?

Why does osteoporosis increase sharply after menopause?

Oestrogen restrains osteoclast activity and keeps bone resorption and formation in balance. After menopause oestrogen falls abruptly, resorption outpaces formation, bone is lost fastest in the first few years, trabeculae thin out and the risk of spine and hip fracture climbs. Prevention means building peak bone mass early - enough calcium and vitamin D, weight-bearing and resistance exercise, no smoking and limited alcohol - while high-risk women can have bone density measured and take antiresorptive drugs under medical guidance.

What is polycystic ovary syndrome (PCOS)?

PCOS is one of the commonest endocrine and metabolic disorders in women of reproductive age, affecting an estimated 8-13% of them according to WHO. Its core features are ovulatory dysfunction (infrequent or absent periods), excess androgens (hirsutism, acne) and a polycystic appearance of the ovaries on ultrasound, and many patients also have insulin resistance, raising their risk of type 2 diabetes and dyslipidaemia. Management starts with lifestyle measures, then adds drugs to regulate cycles, improve insulin sensitivity or induce ovulation, depending on the goal.

What role does insulin play in the body?

Insulin is secreted by pancreatic beta cells and helps blood glucose enter cells (muscle, fat) for energy or storage, thereby lowering blood sugar. Too little insulin or poor response raises blood glucose, as in diabetes.

Why does HbA1c reflect blood sugar over the past few months?

Glucose slowly and irreversibly binds hemoglobin in red cells to form HbA1c, in proportion to average glucose over time. Since red cells live about 120 days, the value reflects roughly the last 2–3 months of control, making it key for diagnosis and monitoring.

Why does chronic dietary iodine deficiency cause diffuse thyroid enlargement (endemic goiter), and which hormonal feedback pathway drives it?

Iodine is the raw material for synthesizing the thyroid hormones T3 and T4. When intake falls, hormone output drops, weakening negative feedback on the pituitary; thyroid-stimulating hormone (TSH) secretion rises persistently and drives prolonged proliferation of thyroid follicular cells, so the gland enlarges diffusely — endemic goiter. This is a compensatory attempt to secure enough hormone with more cells; once salt is iodized the feedback disturbance corrects itself, and most goiters shrink or regress.

Why is levothyroxine best taken on an empty stomach in the morning, several hours apart from calcium or iron supplements?

Levothyroxine is absorbed mainly in the small intestine and dissolves best in acidic gastric juice. Food and cations such as calcium, iron or aluminium, plus acid-suppressing drugs, chelate the tablet contents or slow gastric emptying, cutting absorption by up to 20-30% and making serum levels and TSH fluctuate. Taking it fasting after waking, waiting at least 30 minutes before breakfast and keeping calcium or iron four hours apart keeps absorption steady for precise TSH-guided dosing.

Why does diabetes act like a slow corrosive on blood vessels, damaging even eyes and kidneys?

Chronic high glucose sticks sugars onto proteins and lipids, forming irreversible advanced glycation end products over weeks to months. AGEs cross-link and deposit in vessel walls, stiffening arteries and narrowing lumens, while also triggering receptor-driven chronic inflammation and oxidative stress that ravages the microvascular beds of retina and glomeruli — hence diabetic retinopathy and nephropathy rank among the leading causes of blindness and dialysis. Tight glucose control markedly slows this cascade, which is why long-term HbA1c targets matter.

Why is iodine added to table salt?

The thyroid needs iodine to make thyroid hormone, yet soil and water inland and in mountainous areas contain very little of it. Too little iodine makes the thyroid enlarge as compensation, producing endemic goitre, and deficiency during pregnancy harms fetal brain development, causing intellectual and hearing impairment. WHO and UNICEF therefore recommend universal salt iodization, one of the cheapest and most successful public nutrition measures in history.

Why does an overactive thyroid make people heat-intolerant, sweaty, jittery — hungry yet losing weight?

Thyroid hormone is the body's metabolic throttle: entering the nucleus it upregulates gene expression, boosts mitochondria and uncoupling proteins, and can raise basal metabolic rate by half or more. With excess hormone the engine idles too high — heat production soars causing intolerance and sweating, the heart grows hypersensitive to adrenaline causing palpitations and tremor, and gut transit plus fat-and-muscle breakdown outpace intake so weight falls despite appetite. Treatment accordingly blocks synthesis with drugs, shrinks the gland with radioactive iodine, or removes part surgically.
