Proposed mechanisms include inner-ear microvascular compromise, viral infection, and autoimmune inflammation; hair cells and spiral ganglion neurons shift from reversible to irreversible injury within a short window. Glucocorticoids raise recovery rates by reducing cochlear inflammation and edema, and studies show significantly higher complete-recovery rates when treatment starts within two weeks, especially 72 hours, of onset. After one month, both spontaneous improvement and drug effect fall off sharply.

Episodes are triggered by head-position changes such as turning over in bed, bending down, or lying back; vertigo lasts seconds to under a minute and may come with nausea and torsional nystagmus. In the Dix-Hallpike maneuver the patient is moved quickly from sitting to supine with the head turned 45 degrees and extended, and posterior-canal BPPV produces torsional nystagmus after a brief latency. The nystagmus beats toward the affected ear and often reverses direction on sitting up, which localizes the involved canal.
