What is the difference between the basic reproduction number R0 and the effective reproduction number Rt?
R0 is the average number of secondary cases one infectious person generates in a fully susceptible population with no interventions, a theoretical measure of transmission potential. Rt is the real-time reproduction number at a given moment and already reflects population immunity, behaviour change and control measures, so Rt above 1 indicates a growing epidemic and below 1 a shrinking one, making it the better gauge of intervention effect. Both estimates depend on assumptions about the generation interval, reporting delays and testing coverage, and must be revised as surveillance data accumulate.

How is the herd immunity threshold estimated, and why does measles need about 95 percent coverage?
In a simplified model of homogeneous mixing, the threshold is roughly one minus one divided by R0, so a higher R0 demands a larger immune fraction. Measles R0 is usually estimated at 12 to 18, giving a threshold near 92 to 94 percent, and once imperfect vaccine efficacy and non-uniform contact patterns are considered, two-dose coverage around 95 percent is needed to interrupt endemic transmission. Because immunity is unevenly distributed across regions and groups, local coverage gaps alone can spark outbreaks, so national averages can be misleading.

What is the core idea of the One Health approach?
One Health holds that human, animal and environmental health are interdependent and require coordinated action across sectors. Most emerging infectious diseases originate in animals, and climate change, land-use change, intensive livestock production and wildlife trade all raise the risk of pathogen spillover, so surveillance and control must link veterinary, agricultural, environmental and public health systems. Antimicrobial resistance, food safety and vector-borne diseases are classic One Health issues.

Why can the mRNA vaccine platform respond quickly to newly emerging pathogens?
An mRNA vaccine only needs the gene sequence of the target antigen for in vitro transcription, so there is no need to culture virus or purify protein, and swapping sequences leaves manufacturing and quality control largely unchanged, which compresses the time from sequence to candidate vaccine. Phase 3 trials of COVID-19 vaccines demonstrated high efficacy for the platform and validated lipid nanoparticle delivery with nucleoside modification. Remaining challenges include cold chain requirements, cost and access, and the difficulty of antigen design for other pathogens.

What is Disease X, and why prepare for a disease that does not yet exist?
Disease X is a placeholder used by international public health bodies in priority pathogen lists to denote an as-yet-unknown agent that could cause a serious international epidemic. Including it drives generic preparedness: vaccine and diagnostic platforms whose antigens can be swapped quickly, pre-agreed master protocols for clinical trials, broader genomic surveillance networks, and standing arrangements for supplies and staff. When a genuinely new pathogen appears, response and development then need not start from scratch.

Why is antimicrobial resistance regarded as a major global health threat?
Global burden studies estimate that over a million deaths each year are directly attributable to bacterial resistance, with several million more associated with it, and the burden falls hardest on health systems with fewer resources. Resistance makes common infections, surgery, cancer chemotherapy and intensive care markedly less safe, while development of new antibacterial drugs lags far behind the pace of resistance evolution. Responses include infection prevention and vaccination, antimicrobial stewardship, resistance surveillance networks, and curbing non-therapeutic use in agriculture.

Why do vaccines depend on a cold chain, and why do heat-stable vaccines matter?
Protein antigens, live attenuated viruses and mRNA in vaccines denature or degrade outside their specified temperature range, so immunisation may fail to induce adequate protection, and because such loss of potency is usually invisible, temperature must be monitored from manufacture to the point of use. Most vaccines require 2 to 8 degrees Celsius and some mRNA products need ultra-low temperatures, which is a major bottleneck where electricity and transport are unreliable. Heat-stable formulations, freeze-dried products and vaccine vial monitors substantially improve feasibility and reduce waste in remote settings.
