Why do low-income countries need primary care more than more big hospitals?

The largest share of disease burden is usually preventable or manageable early: antenatal care, childhood diarrhoea and pneumonia, immunisation, hypertension and diabetes control. Primary care networks reach more people at lower cost and catch problems sooner, avoiding expensive admissions. The 1978 Alma-Ata Declaration made primary health care the central strategy, and it underpins universal health coverage today.

How do generic drugs shape global access to medicines?

Patents give innovator drugs a period of exclusivity to recoup R&D costs; once they expire, generics enter and prices often fall to a small fraction of the original. In the early 2000s HIV generics from India and elsewhere cut annual treatment costs from tens of thousands of dollars to the low hundreds, a turning point for scaling up treatment. Generics must pass bioequivalence review so their active ingredient and effect match the original.

Why is antibiotic resistance called a global crisis?

Resistance genes ignore borders: bacteria transfer resistance horizontally on plasmids between species, and travel, food trade and livestock antibiotic use spread resistant strains fast. Misuse anywhere erodes everyone's drug arsenal, while new antibiotic development is slow and commercially unattractive. The response needs prescription stewardship, infection control, agricultural limits and international surveillance together.

Polio has been near eradication for years — why is the last step so hard?

Most infections are asymptomatic — one paralytic case can mean hundreds of silent ones — so surveillance depends on environmental sewage testing and active case search. The live oral vaccine's attenuated virus can, in under-vaccinated communities, mutate into circulating vaccine-derived strains. Remaining endemic areas also face conflict, population movement and distrust, with vaccination teams sometimes at risk.

What are neglected tropical diseases?

A WHO-listed group of diseases affecting mainly low-income regions — schistosomiasis, lymphatic filariasis, trachoma, dengue, leishmaniasis, soil-transmitted helminths and more. They cause disability and poverty yet long attracted little R&D because affected populations have low purchasing power and commercial returns are limited. Mass drug administration, vector control and clean water and sanitation are the main responses.

Why is schistosomiasis spread so tightly linked to specific freshwater snail hosts?

Schistosome larvae (cercariae) must develop inside particular freshwater snail species before they can infect people through the skin in contaminated water. Without the right snail host the parasite cannot complete its life cycle, so snail geography sets the disease's range. Control targets snails, safer water and better irrigation to break transmission.

Why are many infectious diseases called "diseases of poverty"?

Crowded housing eases respiratory spread, lack of clean water and toilets enables faecal-oral transmission, malnutrition weakens immunity, poor access delays diagnosis and treatment, and illness then destroys income — a vicious cycle. That is why the geography of tuberculosis, malaria and intestinal parasites tracks economic conditions closely, and why better housing, nutrition and primary care are themselves infection control.

What are the main health effects of air pollution?

Fine particles (PM2.5) reach the alveoli and even the bloodstream; long-term exposure is linked to asthma, COPD, lung cancer, heart attack, stroke, impaired lung development in children and adverse pregnancy outcomes. WHO attributes millions of premature deaths a year to outdoor and household air pollution, a large share from solid fuels used for cooking and heating. Cleaner fuels, ventilation and transport emission controls are key interventions.

Smallpox is the only eradicated human disease — how was it done?

Key conditions: it infected only humans with no animal reservoir; the rash was distinctive and easy to spot; an effective heat-tolerant vaccine existed; and ring vaccination — rapidly immunising contacts and surrounding communities around each case — broke transmission chains. With international cooperation that crossed Cold War lines, WHO declared eradication in 1980.

Why are clean water and toilets considered among the most effective health investments?

Diarrhoeal disease was long a leading cause of child death, mostly from faecal-oral pathogens. Safe water, sanitary toilets and handwashing facilities simultaneously block cholera, dysentery, hepatitis A, typhoid and many parasites, and reduce malnutrition and stunting. Nineteenth-century London controlling cholera by fixing its water supply is the classic case of public health outperforming individual treatment.

How does stigma around mental illness affect outcomes?

Stigma delays or prevents help-seeking, encourages hiding symptoms, and makes people more likely to drop medication and follow-up; later help-seeking means more functional impairment and higher hospitalisation risk. It also affects employment, relationships and self-image, producing internalised stigma. Using behaviour-describing rather than labelling language, sharing recovery stories, and integrating mental health into primary care are proven ways to reduce it.

Why does tuberculosis treatment take many months of pills?

Mycobacterium tuberculosis grows slowly, has a lipid-rich wall, and can persist in a low-metabolism dormant state inside macrophages and granulomas, so short courses only kill the actively dividing population. Standard regimens therefore combine several drugs for about six months to clear persisters and prevent relapse and resistance. Interrupted treatment is a major driver of multidrug-resistant TB.

Why is vaccination coverage much lower in some places?

Reasons are usually layered: limited supply and funding, cold chain and transport barriers, shortages of health workers, conflict or displacement interrupting services, plus vaccine hesitancy from poor information and low trust. Progress needs both supply-side work (access, cost) and demand-side work (communication, community engagement) — campaigns or donations alone are not enough.

Why hasn't malaria been eliminated?

Malaria is a parasite spread by mosquitoes, so control must tackle parasite, vector and human behaviour at once. Parasites have developed resistance to several drugs, including partial artemisinin resistance in some regions, and mosquitoes to insecticides; the parasite's complex life cycle makes vaccines hard, and current ones offer limited protection. High-burden countries also face stretched health systems. China was certified malaria-free in 2021, showing sustained effort can succeed.
