What are the cellular and molecular mechanisms of aging?

Aging is driven by intertwined mechanisms: telomeres shorten with each cell division and trigger replicative senescence at a critical length; DNA damage accumulation, epigenetic changes, mitochondrial dysfunction, declining proteostasis, and chronic low-grade inflammation accelerate the process together. Cellular senescence itself is a barrier against cancer, but the inflammatory factors senescent cells continuously secrete (the SASP) paradoxically promote tissue aging. These insights have spawned experimental strategies such as clearing senescent cells to slow aging.

What causes hot flushes in menopause?

Falling oestrogen alters the hypothalamic thermoregulatory set point, narrowing the comfortable temperature zone so small rises read as overheating, triggering skin vasodilation and sweating — the sudden wave of heat and flushing. Research implicates hyperactive KNDy neurons in the hypothalamus, now a target for non-hormonal drugs. Treatment choices should be discussed with a clinician.

How are telomeres related to ageing?

Telomeres are repetitive DNA caps on chromosomes that shorten a little at each cell division; at a critical length cells become senescent or die, a hallmark of cellular ageing. But ageing is multifactorial, also involving mitochondrial function, proteostasis, epigenetic change and chronic inflammation. Telomerase can lengthen telomeres, yet excess activity is linked to cancer, so "lengthen telomeres to stay young" is not a straightforward strategy.

Why do human females go through menopause when most animals don't?

In most mammals fertility declines roughly in step with lifespan, whereas human females stop reproducing around mid-life. A widely discussed explanation is the grandmother hypothesis: ceasing reproduction to invest in grandchildren may raise the family's overall reproductive success. A few species with complex societies, such as killer whales and pilot whales, show something similar, lending indirect support.

Why is human childbirth harder than in most other animals?

Human infants have unusually large heads while upright walking favours a relatively narrow, sturdy pelvis — an anatomical tension often called the obstetrical dilemma. The baby must rotate through a series of positions to pass the birth canal. This also helps explain why human newborns are relatively immature, need prolonged care, and why assisted birth is near-universal in human societies.

What does growth hormone do in the body?

Growth hormone, made by the pituitary, is the main driver of bone and soft-tissue growth in children and adolescents, and it also regulates protein, glucose, and fat metabolism. Deficiency in childhood causes growth failure, while excess can cause gigantism; in adults it is more involved in maintaining muscle and regulating metabolism. It is secreted most during deep sleep at night, which is why regular sleep matters for growing children.

Why do many people feel nauseated in early pregnancy?

The leading factors are placental hormones — notably rising hCG and GDF15 — acting near brainstem vomiting centres, together with heightened smell and taste sensitivity. One hypothesis is an evolutionary protective role, steering early pregnancy away from spoiled or toxic foods. It usually eases by the second trimester; seek care if you cannot keep fluids down or lose weight.

What changes does the body go through during puberty?

Puberty is set off by a surge of sex hormones: girls develop breast tissue and start menstruation, boys' testes enlarge and sperm production begins, along with a growth spurt, body hair, and changing body composition. The timing varies widely—girls typically start between 8 and 13 and boys between 9 and 14—and being early or late is usually still normal. Adequate sleep, balanced nutrition, and emotional support help make this transition smoother.

Why does medication need extra caution in pregnancy?

Organs form mainly in the first trimester, when the embryo is most sensitive, and some drugs cross the placenta and affect development — the thalidomide tragedy is the historical warning. Yet taking nothing is also risky: uncontrolled epilepsy, infection, thyroid disease or severe depression harm both mother and baby. The rule is that a clinician weighs risks and benefits — never start or stop medicines on your own.

Why are older adults more prone to vitamin B12 deficiency?

With age, gastric atrophy lowers intrinsic factor and acid, so B12 bound to food is poorly released and absorbed in the ileum—a common “food-cobalamin malabsorption” in elders. Deficiency causes megaloblastic anemia and myelin damage, showing as numbness and memory loss. Checking serum B12 and methylmalonic acid helps detect it early.

Why should folic acid be started before pregnancy, not after?

The neural tube closes around weeks 3–4 after conception, before many people know they are pregnant. Folate supports DNA synthesis and rapid cell division, so daily supplementation started one to three months before conception markedly lowers the risk of neural tube defects such as spina bifida. Dose should follow clinical advice; a family history may call for more.

Which hormones regulate the menstrual cycle?

The menstrual cycle is driven by the hypothalamus-pituitary-ovary axis: during the follicular phase rising estrogen thickens the uterine lining; after ovulation the corpus luteum secretes progesterone, converting the lining to a secretory state ready for implantation. If pregnancy does not occur, the corpus luteum regresses, estrogen and progesterone fall sharply, and the lining sheds as menstruation. The average cycle is about 28 days, though 21–35 days is common.

Why are older adults more prone to dehydration?

With age the thirst sensation dulls, so fluid deficits go unnoticed; the kidneys concentrate urine less well, losing more water; and total body water is lower to begin with. Some also drink less deliberately to avoid night trips to the toilet or because mobility is limited, compounding the risk. In hot weather or illness, drinking on a schedule helps.

What is sarcopenia and why does it matter for older adults?

Sarcopenia is the age-related loss of muscle mass and strength — roughly 3–8% per decade after 40, faster with inactivity, low protein intake or illness. It directly affects walking speed, balance and independence, and is a major risk factor for falls, fractures and disability. Resistance training plus adequate protein intake is the most effective intervention known.

What changes do women experience at menopause, and how can they be managed?

Menopause is the natural decline of ovarian function and permanent end of menstruation, usually between ages 45 and 55. Falling estrogen can cause hot flashes, night sweats, disturbed sleep, and mood swings, and raises the long-term risk of osteoporosis and cardiovascular disease. A healthy diet, exercise, good sleep, and—when appropriate under a doctor's guidance—menopausal hormone therapy (MHT) can all meaningfully improve quality of life.

Why does gestational diabetes develop?

Placental hormones such as human placental lactogen and cortisol reduce maternal insulin sensitivity so more glucose reaches the fetus. If maternal beta cells cannot raise insulin output to match, blood glucose rises. It usually appears in mid-to-late pregnancy and predicts later type 2 diabetes risk, so postpartum glucose follow-up is advised.

Why do teenagers naturally sleep and wake later?

Puberty shifts the body clock later: melatonin release begins about one to two hours later than in childhood, so sleepiness arrives late even though 8–10 hours are still needed. Early school starts clash with this rhythm and produce chronic sleep debt. Dimming light and screens at night and getting morning daylight help shift the clock earlier.

What kind of organ is the placenta?

The placenta is a temporary organ built mainly from fetal-derived tissue that handles oxygen and nutrient exchange, waste transfer, and heavy hormone production (hCG, oestrogen, progesterone) to sustain pregnancy. It is also a selective barrier: it blocks many pathogens and large molecules, yet alcohol, nicotine, some drugs and certain viruses still cross.

What components in breast milk help an infant fight infection?

Breast milk contains secretory IgA that coats the infant gut and blocks pathogen attachment, lactoferrin that competes for iron, lysozyme that breaks bacterial walls, and human milk oligosaccharides that the baby cannot digest but that feed beneficial bifidobacteria. Colostrum is especially concentrated in these. When breastfeeding is not possible, formula also supports healthy growth.

What are child growth charts for, and how do you read them?

Growth charts, such as WHO child growth standards, show the range of weights and heights among healthy children of the same age and sex, often expressed in percentiles. What doctors watch most is the trend in growth velocity—whether a child keeps growing steadily along their own percentile—rather than a single measurement. Small short-term fluctuations are common and nothing to worry about.

Why is alcohol advised against in pregnancy, even in small amounts?

Alcohol crosses the placenta freely, and the fetal liver metabolises it poorly, so fetal blood levels approach the mother's and can disturb neuronal migration and development, causing fetal alcohol spectrum disorders with facial differences, growth restriction and learning and behavioural problems. No safe dose or safe window has been established, so mainstream guidance is no alcohol during pregnancy.

Why are solid foods usually introduced around six months?

Before then, digestive enzymes, kidney handling capacity and swallowing coordination are immature, while breast milk or formula meets all nutritional needs. Around six months, iron and zinc stores start running low and the baby can sit steadily, shows food interest and has a fading tongue-thrust reflex — good timing for iron-rich purées. Individual pace varies, so discuss it with a paediatric clinician.

Why is side-sleeping often recommended in late pregnancy?

Lying flat lets the enlarged uterus compress the inferior vena cava, cutting venous return, which can cause low blood pressure, dizziness and reduced placental flow. Side-lying, especially on the left, relieves this. Most people shift position during sleep, so brief supine periods are not cause for anxiety — follow your antenatal clinician's advice.

What is osteoporosis, and can it be prevented?

Osteoporosis is a metabolic bone disease in which bone mass and microarchitecture are lost and bones become fragile; it is most common in postmenopausal women and older adults, where even a minor bump or a cough can cause a fracture. Prevention centers on building a higher peak bone mass in youth, getting enough calcium and vitamin D, doing weight-bearing exercise, and avoiding smoking and excess alcohol. Once diagnosed, several medications can substantially lower fracture risk.

Why do legs ache during growth spurts?

So-called growing pains typically come in the evening or at night as aching in both legs' muscles rather than swollen joints. Likely causes include muscle fatigue after active days and soft tissue adapting to rapid bone growth — not bone being "stretched". Pain fixed to one side, with swelling, warmth, limping or frequent night waking, should be medically checked.

Why do tiny babies sleep so much?

Newborns sleep about 14–17 hours a day because their brains are rapidly wiring up, and sleep — especially REM sleep — is when those connections form and consolidate; growth hormone is also released mainly in deep sleep. Their stomachs are tiny, so sleep is broken by frequent feeds, and their day-night rhythm has not developed yet.

Why do children's teeth fall out and new ones grow in?

Baby teeth are small teeth for a small mouth. As the jaw grows, the permanent teeth push up from below and the baby tooth's root is gradually resorbed, so it loosens, falls out, and the new tooth comes through. The swap usually starts around age six and finishes by about twelve.

Why do many newborns turn yellow?

Newborn red cells are numerous and short-lived, generating plenty of bilirubin, while the immature liver cannot conjugate and excrete it fast enough, so it accumulates and yellows skin and eyes. Most cases are physiological and fade within about a week. Jaundice that starts very early, rises fast, or comes with lethargy or poor feeding needs prompt assessment.

Why do babies under six months usually need no extra water?

Breast milk and formula are about 85%–90% water and already meet an infant's fluid needs; extra water fills the tiny stomach, cuts milk intake and strains the kidneys. Too much can dilute blood sodium and cause dangerous “water intoxication”. The WHO advises exclusive milk feeding until six months, even in heat, by simply feeding more often.

Why do people get shorter as they age?

Mainly the spinal discs lose water content and thin under compression, which adds up to a few centimetres; declining bone density with slight vertebral collapse and increased spinal curvature also contribute. Weaker postural muscles make people stand less upright. Regular weight-bearing exercise, adequate calcium and vitamin D, and bone density screening help slow it.

Why does acne flare in puberty?

Rising sex hormones, especially androgens, enlarge sebaceous glands and boost oil output, while abnormal shedding of cells plugs the follicle opening; trapped sebum lets Cutibacterium acnes multiply and inflammation follows, producing red spots. Squeezing worsens inflammation and scarring; severe or persistent acne is worth a dermatology visit for proper treatment.

Why is there a soft spot on a baby's head?

That is the fontanelle, a gap where the skull bones have not yet fused. It lets the head mould slightly to pass through the birth canal and leaves room for the fast-growing brain. The front fontanelle usually closes by 12–18 months; gentle everyday touch is safe, but a clearly sunken or bulging spot should be checked by a clinician.

Why do boys' voices break during puberty?

Rising testosterone enlarges the laryngeal cartilage and makes the vocal folds longer and thicker, so they vibrate at a lower frequency and the voice deepens. Meanwhile the laryngeal muscles are still adapting to the new instrument size, causing cracks and squeaks that usually settle within a few months to two years. Girls' folds also lengthen, but less.

Why shouldn't babies under one year old eat honey?

Honey can carry Clostridium botulinum spores. An infant's gut flora is immature and stomach acid weaker, so spores may germinate and produce toxin, causing infant botulism with constipation, weak sucking and floppiness. Ordinary heating does not reliably kill spores, so avoid honey entirely before age one, including foods containing it.

Why do children often run higher fevers than adults?

Children's immune systems react strongly to new pathogens, releasing more fever-inducing cytokines, and their larger surface-area-to-weight ratio with less stable thermoregulation makes temperature swing more. Height of fever does not equal severity — alertness, feeding and breathing matter more. Fever in infants, or unusual drowsiness, warrants prompt medical care.

Why do little kids get sick so often after starting nursery?

Their immune system is still in training. Each new virus teaches the body to recognise and remember it, so next time the fight is faster. Nursery means many children in close contact and many new viruses, so six to eight colds a year is common — and it drops noticeably as they grow.

Why does the adolescent growth spurt usually start earlier in girls than in boys?

Puberty is driven by the hypothalamic–pituitary–gonadal axis; girls typically enter the growth spurt at 9–11 and boys at 11–13 years. Girls start earlier but spurt for a shorter time with smaller total gain, while boys start later yet grow taller for longer, so adult men are on average taller. Timing varies widely, and what matters is that bone age and growth velocity stay reasonable.

Why can tooth decay in baby teeth affect the permanent teeth later?

The permanent tooth buds develop in the jaw beneath the baby teeth; severe decay or early loss of a primary tooth changes chewing forces and space, so the adult tooth may erupt crowded or out of place. Infection of a primary pulp can also reach the underlying bud and cause enamel defects. Brushing from the first tooth, limiting sugar and regular dental visits all matter.
