In suspected acute ischemic stroke, why may early head CT appear completely normal while diffusion-weighted MRI already reveals the lesion?

The difference comes down to how each modality senses water. Within minutes of vessel occlusion, ischemic cells exhaust their energy supply and develop cytotoxic edema that restricts water diffusion, so the infarct core turns bright on DWI almost immediately and becomes delineated within hours. CT depends on density differences that barely change at onset, often needing around twenty-four hours to become visible. DWI thus anchors hyperacute diagnosis and reperfusion decisions, while CT mainly serves rapid exclusion of intracranial hemorrhage.

MRI uses no X-rays, so how does it turn the inside of the body into pictures?

It relies on resonance of the body's abundant hydrogen nuclei inside a magnetic field. A strong magnet aligns the randomly oriented proton spins; a radiofrequency pulse tips them out of equilibrium, and as they relax back they emit faint RF signals. Because tissues differ in water content and relaxation speed, their signal strengths differ too; gradient fields encode spatial position and a computer reconstructs cross-sectional images - entirely without ionizing radiation.

Why can low-dose spiral CT reveal small lung cancers that an ordinary chest X-ray misses?

The gap stems from how each technique forms images. A chest X-ray exposes the whole thorax at once, so shadows of lung, heart and ribs overlap and hide nodules only a few millimeters wide. CT scans slice by slice and reconstructs thin cross-sections free of overlap, resolving tiny solitary nodules and catching tumors at a still-curably localized stage. A large screening trial showed that low-dose CT in high-risk smokers cuts lung cancer mortality by about twenty percent.
