How did antiretroviral therapy turn HIV/AIDS from fatal to manageable?

Barré-Sinoussi and Montagnier isolated HIV in 1983; combination antiretroviral therapy (HAART) blocking reverse transcriptase and protease can suppress viral load to undetectable, giving near-normal life expectancy. AIDS shifted from inevitably fatal to a manageable chronic disease.

What is phage therapy, and why is interest reviving?

Bacteriophages are viruses that infect bacteria specifically, recognizing particular strains, replicating inside them and lysing them. They were tried against infections before antibiotics became widespread, and now, with multidrug-resistant bacteria spreading, phage therapy is being revisited in compassionate-use cases and clinical trials as a complementary option. Its attractions are high specificity that spares normal flora and the ability to co-evolve as bacteria change. The hurdles are a narrow host range requiring careful matching, clearance by the immune system, and the lack of standardized manufacturing and regulatory pathways - so it remains investigational rather than routine.

How was Helicobacter pylori proven to be the real cause of stomach ulcers?

Marshall and Warren isolated a curved bacterium from ulcer patients' gastric mucosa in 1982 and even swallowed it themselves to induce gastritis, proving its link to ulcers and overturning the old 'stress and acid' view. They won the 2005 Nobel Prize.

Why do bacteria become antibiotic-resistant, and how does resistance spread so fast?

Resistance arises from random mutation or acquired resistance genes, then survives under drug selection; horizontal gene transfer via plasmids, transduction, and conjugation lets resistance genes spread quickly across species. The WHO calls it a major global health threat.

What are prions, and why did they challenge the idea that infectious agents need nucleic acid?

Prions are misfolded proteins that induce normal homologous proteins to misfold and aggregate, causing spongiform degeneration of the brain. Prusiner proposed this nucleic-acid-free infectious mechanism in 1982 and won the 1997 Nobel Prize.

How does artemisinin treat malaria, and why was Tu Youyou's work important?

Artemisinin comes from sweet wormwood and, inside the parasite-laden red cell, generates reactive oxygen species that damage parasite proteins, killing rapidly and working against resistant strains. Tu Youyou drew extraction insight from ancient texts; the work won the 2015 Nobel Prize.

How did the discovery of penicillin change medical history?

How does a virus get into a human cell?

A virus's attachment protein must first bind a specific receptor on the host cell membrane, like a key fitting a lock: the SARS-CoV-2 spike binds ACE2, HIV binds CD4 with CCR5, and influenza binds sialic acid on the cell surface. After binding, the virus either fuses directly with the membrane or is taken up in a vesicle before releasing its genetic material. Where those receptors occur in the body determines a virus's tissue preference and symptoms, which is why blocking receptor binding is a central target for antiviral drugs and vaccine design.

What is the fundamental difference between bacteria and viruses?

Bacteria are complete single-celled organisms that metabolize and reproduce on their own and can be killed by antibiotics. Viruses lack independent metabolism and must hijack a host cell's machinery to replicate, so antibiotics do not work and we rely on antivirals or vaccines.
