What do the amyloid-beta hypothesis and tau pathology each explain in Alzheimer disease?

The amyloid hypothesis holds that abnormal cleavage of amyloid precursor protein yields amyloid-beta 42, which aggregates into plaques and triggers glial inflammation and synaptic injury; tau pathology involves hyperphosphorylated microtubule-associated tau forming neurofibrillary tangles whose spread correlates more closely with cognitive decline. The prevailing view is that amyloid deposition initiates the disease in the preclinical stage, while tau propagation and neuronal loss set the pace of symptoms. This helps explain why anti-amyloid drugs are aimed at early disease and offer little benefit later on.

How is the brain glymphatic pathway related to sleep and neurodegeneration?

The brain lacks conventional lymphatic vessels, so metabolic waste is largely cleared along perivascular spaces through exchange between cerebrospinal and interstitial fluid, a route that depends on AQP4 water channels on astrocyte end-feet. Animal studies show that during sleep the interstitial space expands and exchange becomes much more efficient, increasing clearance of solutes including amyloid-beta, which suggests chronic sleep deprivation could favour protein accumulation. Quantitative human evidence is still emerging, so it would be premature to claim that improving sleep alone prevents neurodegenerative disease.

Why does long-term levodopa lead to wearing-off and dyskinesia?

As nigral neurons continue to die, the striatum loses its capacity to store dopamine and release it steadily, so fluctuations in plasma levodopa are transmitted directly to synapses and each dose lasts less time, which is wearing-off. Repeated pulsatile receptor stimulation then drives maladaptive plasticity in downstream striatal signalling pathways, producing the excessive involuntary movements called dyskinesia. Management smooths stimulation using smaller, more frequent doses, extended-release formulations or COMT and MAO-B inhibitors, with deep brain stimulation considered when medication adjustment fails.

Can lifestyle changes really lower the risk of dementia?

Large evidence reviews estimate that addressing modifiable factors at the population level, including uncorrected hearing loss, low education, hypertension, smoking, obesity, diabetes, physical inactivity, social isolation, depression, excessive alcohol, air pollution, head injury, visual loss and high LDL cholesterol, could in theory prevent or delay around forty-five percent of dementia. These are population attributable estimates, so no individual is guaranteed protection, but managing the same factors also benefits cardiovascular and cerebrovascular health. Evidence suggests the potential gain is larger when intervention begins in midlife.

Does mild cognitive impairment always progress to dementia?

Mild cognitive impairment means measurable decline in memory or another cognitive domain while everyday function is largely preserved. Follow-up studies find that somewhat over ten percent of people with MCI progress to dementia each year, yet a substantial share stay stable long term and some improve once reversible contributors such as depression, sleep apnoea, hypothyroidism, vitamin B12 deficiency or medication side effects are corrected. It is generally advised to look systematically for these reversible causes and to reduce risk by controlling blood pressure and glucose, exercising regularly and staying cognitively and socially active.

Why do people with Parkinson disease develop tremor and slowed movement?

Dopaminergic neurons in the substantia nigra pars compacta degenerate, so striatal dopamine falls and the basal ganglia-cortical loops lose balance between initiating and suppressing movement, producing rest tremor, rigidity, bradykinesia and postural or gait problems. Obvious motor signs usually appear only after more than half of these neurons are lost, so the underlying process has often run for years before diagnosis. Non-motor features such as reduced smell, constipation and shouting or thrashing during sleep frequently precede the motor symptoms.

What is the blood-brain barrier and why does it make treating brain disease so hard?

The blood-brain barrier is formed by tight junctions between brain capillary endothelial cells, the basement membrane and astrocyte end-feet; it blocks large molecules and most hydrophilic substances while efflux transporters actively pump foreign compounds out, keeping the central nervous system environment stable. Drugs generally need low molecular weight, high lipid solubility or a specific carrier to cross, so many large molecules that work in the laboratory never reach their target. Researchers are testing delivery strategies such as focused-ultrasound opening, receptor-mediated transport and intranasal administration.

Do anti-amyloid antibodies actually cure Alzheimer disease?

Phase 3 trials show these antibodies markedly clear amyloid plaques and slow decline on cognitive and functional scales by roughly a quarter over 18 months in early disease, which means slowing progression rather than reversal or cure. Amyloid pathology must be confirmed by PET or cerebrospinal fluid testing before treatment, and MRI monitoring is needed for amyloid-related imaging abnormalities, that is brain oedema and microhaemorrhage. It is generally recommended that such therapy be started only in centres able to perform biomarker testing and imaging follow-up, after weighing benefits against risks.
