What are the beta-amyloid plaques in Alzheimer's disease?
Beta-amyloid derives from abnormal cleavage of the amyloid precursor protein and aggregates into plaques that provoke inflammation and tau tangles, progressively harming synapses and memory. Hardy and Selkoe's 'amyloid cascade' is the leading framework, though most plaque-clearing trials have struggled.

How does the blood-brain barrier protect the brain yet keep drugs out?
Brain capillary endothelial cells are sealed by tight junctions and wrapped by astrocyte end-feet, letting through only small lipophilic molecules, oxygen, and specific transporter substrates. This tight filter blocks most toxins and drugs, making CNS delivery a major hurdle.

What is the current scientific explanation for how general anesthesia causes unconsciousness?
Anesthetics mostly enhance inhibition via receptors like GABA_A or block excitation via NMDA, altering large-scale neural synchrony and disrupting thalamocortical information integration. The precise mechanism is still unresolved, but network-level studies partly explain it.

Is the placebo effect a real physiological response or just psychology?
Placebos can trigger genuine neuroendocrine changes—such as endogenous opioid release—in pain and Parkinson's settings, not mere imagination. Research shows they are shaped by expectation, conditioning, and the clinician-patient interaction.

How do neurons pass signals across a synapse?
The electrical impulse reaching the axon terminal triggers calcium influx, causing synaptic vesicles to release neurotransmitters into the cleft; these bind receptors on the next neuron and trigger a new potential. This chemical relay lets signals pass flexibly between cells and be modulated.

What irreplaceable role does sleep play for the brain?
During sleep the brain clears metabolic waste such as beta-amyloid and consolidates the day's memories while reshaping connections. Tononi and Cirelli's synaptic-homeostasis hypothesis holds that slow-wave sleep proportionally downscales daytime synaptic strengthening to avoid overload.

What does dopamine actually do in the brain?
Dopamine both controls movement (the nigrostriatal pathway) and lies at the core of reward and motivation, driving the 'wanting' of beneficial stimuli. Carlsson showed its loss causes Parkinsonian symptoms, earning the 2000 Nobel Prize.

Why does levodopa improve Parkinson's disease symptoms?
Parkinson's arises from loss of midbrain dopamine neurons, causing slowness, tremor, and rigidity. Levodopa crosses the blood-brain barrier and is converted to dopamine in the brain, replacing what is missing, so it remains the core symptomatic drug.
