Can HIV infection be controlled today?
Yes. HIV reverse-transcribes and integrates into the CD4+ T-cell genome, so the immune system alone cannot clear it, but antiretroviral therapy (ART) blocks viral replication at multiple steps—reverse transcription, integration, and protein maturation. With consistent treatment, viral load falls to undetectable and CD4 cells recover. Undetectable means untransmittable (U=U): people with HIV can live long, healthy lives and do not transmit the virus sexually or to their babies.

Why is tobacco still called the world's leading preventable cause of death?
About 8 million people die each year from tobacco-related disease; smoking is the top shared risk for lung cancer, COPD, coronary disease and stroke, and secondhand smoke harms non-smokers. The GBD study ranks tobacco among the leading causes of disability-adjusted life years. Because the harm is vast yet clearly preventable, tobacco control is among the most cost-effective public-health actions.

What is multidrug-resistant tuberculosis (MDR-TB)?
MDR-TB means the bacteria are resistant to both isoniazid and rifampicin—the two most effective first-line drugs—arising mainly from irregular treatment (stopping early, single-drug exposure) or infection with a resistant strain. Treatment requires second-line drugs for 18 months or more, with more side effects, lower cure rates, and higher costs. Completing the full supervised course is the key to preventing resistance, while transmission of resistant strains worsens the burden further.

Why do some vaccines require booster doses?
Initial vaccination generates memory B and T cells, but antibody titers from inactivated-type vaccines wane over time. A booster is like a simulated re-exposure: it re-activates memory cells, quickly raising antibody levels, drives germinal-center reactions for affinity maturation, and expands long-lived bone-marrow plasma cells to greatly extend protection. This is the clinical application of immunological memory—the same principle as the body's response to a real pathogen re-encounter.

Why can a second dengue infection be more dangerous?
Dengue virus has four serotypes. Antibodies from a first infection neutralize the same serotype, but sub-neutralizing antibodies against other serotypes can cause antibody-dependent enhancement (ADE): antibody-virus complexes are taken up more readily by macrophages via Fc receptors, allowing massive viral replication inside them and triggering a fiercer immune reaction—raising the risk of severe dengue (dengue hemorrhagic fever/shock syndrome). This is why dengue vaccines must induce balanced protection against all four serotypes.

What steps does a vaccine go through from research to market?
Vaccine development begins with preclinical research — cell and animal studies of safety and immunogenicity — followed by clinical trials in phases: I (small groups, safety and dosing), II (immune response and dose range), and III (large trials of efficacy and safety). The data are then submitted for regulatory review and approval, and after market authorization continuous phase IV surveillance tracks rare adverse events long-term. Emergency use authorization accelerates the process but still requires the available evidence to show benefits outweigh risks.

Why is the randomized controlled trial considered the gold standard for treatment evidence?
Randomization balances treatment and control groups on both known and unknown factors; blinding keeps doctors and patients unaware of who gets what, avoiding expectations; and placebo controls rule out natural recovery. This makes it reasonably safe to attribute outcome differences to the drug itself, which is why rigorous RCTs are required before new medicines reach the market.

Why does hepatitis B control rely on birth-dose vaccination and mother-to-child blocking rather than treatment?
Hepatitis B spreads mainly from mother to newborn, and perinatal infection readily becomes chronic, progressing to cirrhosis and liver cancer. A birth-dose vaccine within 24 hours plus hepatitis B immunoglobulin cuts this main chain far more cheaply than lifelong antivirals. The WHO therefore makes “vaccinate at birth” the core of hepatitis B elimination.

Why did WHO list "vaccine hesitancy" as a global health threat?
Vaccine hesitancy is delaying or refusing vaccination despite availability, often fueled by misconceptions and misinformation. Falling coverage weakens herd immunity, allowing preventable diseases like measles to return. Most concerns can be resolved with patient, transparent communication.

Why is HPV testing more sensitive than the Pap smear for cervical cancer screening?
Almost all cervical cancers follow persistent high-risk human papillomavirus infection, so testing viral DNA detects risk at the precancer stage and is more sensitive than reading cell shapes under a microscope. The WHO 2021 guideline recommends HPV testing as the primary screen, with positive results triaged further. This finds high-risk women earlier and more reliably.

What is 'herd immunity'?

Why do heated tobacco products still contain carcinogens and nicotine?
Heated tobacco devices do not burn leaf but heat it to about 350°C, still releasing tobacco chemicals including formaldehyde, acrolein, benzo[a]pyrene and nicotine. The WHO states these products are not harmless, emit toxic aerosols and sustain nicotine dependence, and should not be seen as safe alternatives.
How is a vaccine's real-world effectiveness evaluated?
Clinical trials measure vaccine efficacy under controlled conditions, while its performance in real populations is called effectiveness, and the two are not identical. Researchers compare disease rates in vaccinated versus unvaccinated people to estimate protective effectiveness while handling confounding such as healthy-user bias.
Why do we need an influenza vaccine nearly every year?
Influenza viruses mutate quickly: surface hemagglutinin and neuraminidase constantly drift, so last year's vaccine strains may not match this year's circulating viruses. Every year WHO predicts the next season's strains from global surveillance and updates the vaccine composition. Getting the seasonal vaccine each year is what keeps protection matched to the circulating strains.

Why doesn't correlation imply causation?
Two phenomena occurring together may share a common third factor (confounding), the "outcome" may have preceded the "exposure" (reverse causation), or it may be pure chance. Establishing causation requires ruling out confounding, confirming the correct time order, and supporting biological plausibility. RCTs remove confounding by randomization; observational studies approach causal conclusions with multivariable regression, propensity scores, or even Mendelian randomization.

Why does high vaccine coverage protect people who are not vaccinated?
Every infectious disease has a transmission power — how many people one case typically infects. When enough people in a population are immune through vaccination or past infection, the pathogen cannot find enough susceptible hosts, transmission chains break, and even unvaccinated people are protected. This matters especially because infants, the elderly, and immunocompromised individuals often cannot be vaccinated.

What is the difference between cohort and case-control studies?
A cohort study groups people by exposure and follows them over time to observe outcomes, so it can directly estimate incidence and risk with higher evidentiary strength—but it is slow and expensive. A case-control study starts with people who already have the disease and looks back at their past exposures, comparing them with healthy controls; it is fast and cheap but prone to recall bias. Both are observational and must be interpreted with confounding in mind.

What is universal health coverage?
Universal health coverage means everyone can obtain the quality health services they need without suffering financial hardship in paying for them. It has two dimensions: service coverage - prevention, treatment, rehabilitation and palliative care - and financial protection, achieved through prepayment and risk-pooling mechanisms such as taxation or social insurance that shrink out-of-pocket spending. It forms Sustainable Development Goal target 3.8, and WHO regards strong primary health care as the most cost-effective route to achieving it.

How is tuberculosis transmitted and treated?
Tuberculosis is caused by Mycobacterium tuberculosis and spreads mainly by droplets from coughing or sneezing people with active pulmonary TB, affecting the lungs most often. In most infected people the immune system holds the bacteria in a latent state; about 5–10% develop active disease in their lifetime. Active TB requires a standard combination regimen for at least six months; irregular treatment is the main cause of drug resistance.

What do sensitivity and specificity of a screening test mean?
Sensitivity is the proportion of truly diseased people correctly detected—higher means fewer missed cases. Specificity is the proportion of truly healthy people correctly ruled out—higher means fewer false alarms. The two usually trade off against each other, so the balance depends on the goal: to avoid missing serious disease you favor sensitivity; to reduce anxiety from false positives you favor specificity.

Healthcare-associated infections are infections acquired during a hospital stay, commonly urinary tract, surgical site, ventilator-associated pneumonia, and bloodstream infections. The more invasive the procedures and the longer the stay, the higher the risk. Hand hygiene is the single most basic and effective measure, along with proper instrument sterilization, prudent antibiotic use, and infection surveillance.

How is dengue spread, and what are its main features?
Dengue is caused by dengue virus and spread by Aedes mosquitoes, mainly Aedes aegypti and Aedes albopictus. After an incubation of a few days, patients develop sudden high fever with severe headache, pain behind the eyes, muscle and joint aches, and a rash. Most recover within about a week, but a minority progress to severe dengue with bleeding and shock signs that need urgent care. Mosquito control is the most important preventive measure.

Are e-cigarettes safer than conventional cigarettes?
E-cigarettes do not burn tobacco, so some combustion products are reduced - but that is far from harmless. Their aerosol contains nicotine, which is highly addictive and can harm adolescent brain development, along with ultrafine particles, carbonyl compounds and metals, and long-term health effects are still being studied. Some evidence suggests they may help certain adult smokers quit when combined with professional support, yet WHO emphasizes their clear harms to young people and non-smokers and calls for strict regulation and bans on youth-oriented marketing. People who have never smoked should not start on the grounds that vaping is safer.

Solid fuels and kerosene burning incompletely in poorly ventilated rooms generate high concentrations of PM2.5, carbon monoxide and polycyclic aromatic hydrocarbons, with particle levels far above outdoor air quality limits. WHO estimates household air pollution causes about 3.2 million deaths a year, chiefly through childhood pneumonia, chronic obstructive pulmonary disease, lung cancer, ischaemic heart disease and stroke, with women and children hit hardest because they spend most time near the stove. Switching to clean fuels such as LPG or electricity, or to improved stoves, plus proper venting, is the most direct and effective intervention.

What are passive and active immunity?
Active immunity is made by the body itself, like after vaccination, and lasts long. Passive immunity infuses antibodies directly, like tetanus antitoxin, acting fast but briefly. Each has its role in post-exposure and outbreak control.

How does malaria infect people, and how is it diagnosed and treated?
Malaria parasites enter the blood through the bite of female Anopheles mosquitoes, multiply first in the liver, then invade red blood cells in repeated cycles that cause periodic chills, fever, and sweating. Diagnosis relies on blood-smear microscopy or rapid tests. Artemisinin-based combination therapy (ACT) is the standard treatment; mosquito control—bed nets and repellents—is the key to prevention.

A worker with 8 years of exposure to steady noise at 85 dB(A) shows markedly elevated thresholds at 3000, 4000 and 6000 Hz while speech frequencies remain essentially normal. Can occupational noise-induced deafness be excluded on this basis, and what are the management principles once confirmed?
Noise-induced hearing loss strikes the high frequencies first and speech frequencies may stay normal early on, so normal speech thresholds do not rule it out; assessment follows the diagnostic standard using age- and sex-corrected binaural high-frequency averages. Once confirmed, the worker must be removed from noise exposure, fitted with hearing aids when loss is marked, and kept under regular audiometric review. Employers must also arrange in-service and post-exposure surveillance according to occupational health surveillance protocols.

How is the workplace noise occupational exposure limit of 85 dB(A) defined, and what obligations follow for employers?
China's GBZ 2.2 sets the noise limit at an equivalent continuous sound level of 85 dB(A) averaged over a 40-hour working week, with instantaneous levels never exceeding 115 dB(A). Employers must measure noise periodically, provide hearing protectors for posts exceeding the limit, and arrange pre-placement and in-service occupational health examinations. An 8-hour equivalent level of 80 dB(A) or above already constitutes hazardous noise exposure requiring hazard disclosure and enrollment in a hearing conservation program.

Why does the WHO advise adults to eat less than 5 g of salt per day?
Excess salt raises blood volume and pressure, and long-term high sodium is a leading dietary driver of hypertension and the resulting stroke and heart disease. Keeping daily salt under 5 g cuts cardiovascular events across populations. Reducing salt means watching soy sauce, pickles and processed foods, not just the shaker.

Why are mouldy peanuts and corn prone to aflatoxin and linked to liver cancer?
In warm damp conditions, Aspergillus on peanuts and corn makes aflatoxin B1, an IARC group 1 carcinogen that the liver metabolises into DNA-damaging forms.
Why does oral rehydration therapy alone sharply cut cholera deaths?
Cholera kills mainly through dehydration and electrolyte loss from profuse diarrhea and vomiting, not the bacterium itself. Oral rehydration salts use the glucose–sodium co-transport so the gut absorbs sodium and water efficiently, correcting dehydration fast. Timely ORT drops cholera case-fatality from over 30% to under 1%, making it one of the most cost-effective treatments.

Why do helmets and seat-belts save lives?
In a crash the body decelerates abruptly and the head and internal organs take enormous force. A helmet's crushable liner stretches that deceleration by a few milliseconds and spreads the load over a wider area, while a seat-belt holds the body in the seat, stops it striking the steering wheel or being thrown out, and channels force through the strongest bones. WHO data show that wearing a helmet correctly cuts a motorcyclist's risk of death by about 42% and of head injury by about 69%, seat-belts reduce front-seat occupant deaths by roughly 45-50%, and child restraints cut deaths in young children by more than 60%.

Why is an injection needle thrown away after a single use?
A needle touches blood once it goes through the skin, so reusing it or sharing it between people can pass on hepatitis B, hepatitis C or HIV in the traces of blood left behind. A used tip is also blunter and may be contaminated, making a second use more painful and more likely to cause infection. The rule is therefore one needle and one syringe per person, dropped straight into a dedicated sharps box afterwards, and WHO promotes auto-disable syringes that cannot be used twice.

What is herd immunity?

Why do doctors tap your knee? What does the reflex reveal?
Tapping the ligament sends sensors racing to the spinal cord, which orders the leg to kick without consulting your brain — too fast to stop. The reflex arc tests neural wiring: too weak or strong flags specific problems. Bodies run many such automatic emergency designs where speed beats deliberation.
