What are the advantages and limits of laparoscopic (minimally invasive) surgery?
Laparoscopy inserts a camera and instruments through a few small incisions and inflates the abdomen with carbon dioxide to create working space. Compared with open surgery it means smaller wounds, less pain, shorter hospital stays, fewer wound infections and hernias, and a faster return to normal activity. Its limits are costly equipment and training, a long learning curve and reduced tactile feedback; it is unsuitable for dense adhesions, heavy bleeding or haemodynamically unstable patients, and converting to an open operation mid-procedure is a safety decision rather than a failure.

Why must you fast before surgery?
General anaesthesia suppresses the protective reflexes of the throat, so if stomach contents reflux they can be inhaled into the lungs and cause severe chemical pneumonitis - one of anaesthesia's important risks. Modern guidelines do not demand nothing at all from the night before: clear fluids are generally allowed until two hours before, breast milk until four hours, and a light solid meal until six hours. Excessively long fasting brings dehydration, low blood sugar, thirst and distress, especially in children and older adults, so follow the specific instructions of your anaesthetist.

What is the fundamental difference between general and local anaesthesia?
General anaesthesia uses drugs acting on the central nervous system to produce reversible unconsciousness, amnesia, analgesia and muscle relaxation; the patient loses the ability to protect their own breathing, so the airway must be secured, ventilation supported and vital signs monitored throughout. Local and regional techniques - infiltration, nerve blocks, spinal or epidural anaesthesia - mainly block sodium channels so nerve signals from one region cannot get through, and the patient can stay awake. The choice depends on the site and length of surgery, comorbidities and preference, and because the risk profiles differ, an anaesthetist weighs them together.
