In solid organ transplantation, why does the CMV serological combination of donor-positive and recipient-negative (D+/R-) carry the highest infection risk, and how is it prevented in clinical practice?

Latent cytomegalovirus within the donor organ reactivates in a recipient who lacks CMV-specific immunity, producing primary infection, the most severe pattern, with possible pneumonitis, enteritis and heightened rejection risk. Prevention consists of oral valganciclovir for three to six months depending on the organ, scheduled plasma viral-DNA surveillance, use of leukocyte-reduced blood products, and avoidance of overly intense immunosuppression, which together markedly lower early CMV disease incidence.

After receiving someone else's organ, why do patients usually need long-term, often lifelong, immunosuppressive drugs?

Donor organs carry HLA antigens that the recipient's immune system reads as foreign; T cells become activated through direct and indirect pathways of allorecognition and then attack the graft's vessels and tissue. Reliable, lasting immune tolerance cannot yet be routinely achieved in humans, and self-directed dose cuts or stopped medication readily trigger acute rejection or chronic graft loss. Most recipients therefore continue maintenance immunosuppression for life, continually weighing anti-rejection protection against infection and cancer risks.

Why must solid organ transplant recipients taking tacrolimus have regular blood trough-level checks rather than staying on a fixed dose?

Tacrolimus has a narrow therapeutic index, and intestinal absorption and CYP3A-mediated metabolism vary enormously between individuals, so the same dose yields blood levels differing severalfold. Levels too low fail to suppress T-cell activation, inviting acute rejection; levels too high cause nephrotoxicity, tremor and other neurotoxicity, and raise glucose. Regular trough measurements with doses adjusted to liver and kidney function balance rejection control against toxicity; targets start higher right after transplantation and step down over time.
